WE NEED THE HELP OF STUDENTS IN THE SCHOOL DISTRICT!

The overall goal of Oklahoma’s GEAR UP project is to more effectively prepare students for success in college. By providing information, financial assistance, and support for developing academic skills, GEAR UP can make positive impact on the future of the district’s students after high school. We encourage you to participate in the survey. Please note, however, that participation is strictly voluntary. The survey is anonymous and does not ask for your name. The information from the survey will be used by the school district to meet GEAR UP goals and will be compiled so that no single survey form will be identified. District survey information may be combined with survey information from other school districts in Oklahoma in summary form.

If you have problems or need assistance with the survey, please contact:
Randy McCrary
Phone: (405) 225-9193

Use the tab key to navigate through the form. Use the "Esc" key or "Reset" button to clear the form.

Use the "Submit" button at the bottom of survey to submit your completed form.
